Cancer Support for Ontarians
CareChart (CAREchart@home), a subsidiary of Bayshore HealthCare Ltd., is an afterhours virtual support program for patients undergoing cancer treatment. Sponsored by Ontario Health and Cancer Care Ontario, this program provides symptom management support for all oncology patients receiving systemic and radiation therapy when their local treatment centre is closed. Between their cancer treatment centre and CareChart, patients have access to 24-hour care, 365 days a year to help them manage their symptoms while remaining at home.

CareChart Palliative Care
A medical diagnosis can be emotionally distressing and physically challenging for both patients and their families. CareChart nurses understand the unique needs and challenges of palliative care and are there to offer compassionate guidance and support through one of life’s most difficult times. Piloted in the northwest region of Ontario, CareChart Palliative Care prioritizes quality of life, providing enhanced support and expert advice to complement the healthcare system and ensure patients and their families receive the right treatment, care and symptom management plan.
